A Department of Transportation (DOT) physical examination, sometime called CDL exams (Commercial Drivers License), are conducted by a licensed “medical examiner.” The term includes, but is not limited to, doctors of medicine (MD), doctors of osteopathy (DO), DOCTOR OF CHIROPRACTIC(D.C.) physician assistants (PA) and advanced nurse practitioners (ARNP).

2. Who will perform the DOT physical examination?

My name is Dr. Lonney Edwards, D.C.  I a Doctor of Chiropractor and I will be performing your examination. I am also certified by the National Academy of DOT Medical Examiners.

3.  How long does it take to complete the DOT examination?

Providing that no complicated medical issues arise during the course of your evaluation, it can take about half an hour.

4.  How should I prepare and what should I bring with me to the examination?

Although no special preparations are required, do maintain a healthy lifestyle.  Proper diet and exercise, and avoiding tobacco use will help you achieve a more ideal body weight and blood pressure.

If you wear corrective lenses (eyeglasses or contact lenses) bring them with you for the examination.  If you recently had an eye exam performed by an ophthalmologist or optometrist bring the exam results with you.  Also, if you use a hearing aid bring them along as well for the examination.

Of course, do not forget to bring your driver’s license.

5.  What to expect during the examination?

First of all relax.  If this is a new certification we will do our best to help you with any questions and concerns that you may have.  Be as thorough as you can in filling out your medical questionnaire.  Explain any yes answers in detail and list any medications that you are taking along with the names of your treating physicians. Sign your history form.

6.  Do I need to bring any of the medical forms with me?

No.  The medical form, Form 649-F( 6045) Medical Examination Report For Commercial Driver Fitness Determination will be provided by our office.  If you like, you can download the form to fill out before your appointment from our web page or from the link provided to the FMCSA (Federal Motor Carrier Safety Administration) website.  Our office will also provide the Medical Examiner’s Certificate to our qualified drivers after the examination.

7.  Will I have to take a urine drug test in order to get my Medical Examiner’s Certificate”

No.  Drug screening is not required in order to obtain your Medical Examiner’s Certificate.  Unless specifically requested it is not routinely performed. 8.  If a Urine Drug Screen is not required then what do you test for when you take my urine?

We perform the required urine dipstick test to evaluate for protein, blood, glucose (sugar), and specific gravity (concentration of particles in urine).  It helps screen for disease such as diabetes, kidney problems, etc.

9.  Will the Medical Examiner’s certificate be issued the same day?

Yes.  Qualified Drivers are issued their Medical Certificate the same day.

10.  Are walk-ins allowed and If I want to make an appointment how do I do so?

Yes.  Walk-Ins are welcome but please check our business hours that are listed in the Contract pages .   To avoid waiting it is better to call us at  800.542.2225
